What Causes A Sore Spot In The Anal Region?

My brother has a very sore hole at the top in between his butt crack. It s been draining puss and bleeding. It s been hard for him to sit down or lay on it. It started the size of a pencil tip now u can almost put your pinkie finger in it. He s been wearing women s pads to catch the blood & puss. He s wondering how he should clean it he s scared to get soap in the hole. What is this? He said it has happened before abt 6 year s ago but it went away

COnsidering your description it seems like there is so calle dpylonidal sinus or perianal abscess,fistula. This is probably sinus which is small canal under the skin that may become inflammed so it can be treated by steroid creams ut surgery is usually the only permanent solution and I recommend it.
