General Surgeon 's  Response
Hi,

Noted your history of having second Mirena insertted abotut a year ago and have pain in lower abdomen for the last one month.
Hence Mirena IUD does not look to be related to your lower abdominal pains.
Hence I would advise you the following:
Consult your Physician for clinical check-up.
Tests of stool, blood and urine.
Ultrasonography of abdomen.
Other tests to know the cause of pain in lower abdomen.
Prescription based treatment as per the reports and clinical evaluation.
If needed you can have a second opinion of a Gastroenterologist and have further management.
Also take an opinion of a Gynecologist about your menstrual problems.

Hope I have answered your query. Let me know if I can assist you further.

Regards,
Dr. T. Chandrakant, General Surgeon
