We have to evaluate the patient on the following lines even if the symptoms are mainly in the left side:
1. Is the pain related to food? If it is relieved by food, it is most likely
duodenal ulcer.
2. If there is tenderness under the costal margin on deep inspiration, one can think of
chronic cholecystitis.
3. If liver is palpable & tender, look for jaundice (infective hepatitis), intercostal tenderness (amoebic hepatitis).
4. If
gall bladder is palpable with or without jaundice, refer to a surgeon for investigations.
PS. If pain is not relieved completely ask for investigations: gastroscopy, stool examination, gastric analysis.
5. If symptoms are only present on left side then according to the region one may suspect Renal pain,ureteric colic and may ask for Urine examination, X-ray KUB &
Ultrasonography.
6. Till then take:
. Tablet
Albendazole 400 mg (broad spectrum anthelminthic)
. Tablet Metronidazlle 400 mg thrice daily for 7 days
. Tablet Anafortan (chamylofin)
. Tablet Ranitidine 150 mg twice daily
