Even though a clinical assessment is essential for a diagnosis of your condition, from the history it seems to be related to the inflammation of the
gall bladder with calculi in it, or could be due to the visible
peristalsis of the distended intestines due to any kind of obstructive pathology lower down. If I were your doctor, I would go for a physical examination of your wife's abdomen followed by some
blood test like a CBC, blood sugar, LFT, serum
amylase and lipase, and an
ultrasound of the abdomen to know the status of the gall bladder,
pancreas and to rule out a gall stone disease. Further management would depend on the clinical result and reports of the related investigations. Till then , you could go for an anti spasmodic and a
proton pump inhibitor drug for a symptomatic relief.
