What Causes An Infected Ulcer On Foot After An Injury?
I'm currently a 3rd year medical student and this case was presented to me today. I'll like your opinion; 45 years old male patience who on his job got injured on his ankle with a rusty steel. The next day he started with flu-like symptoms, fever. A few days later his ankle inflamed, and he notice an ulcer on his foot which was infected. He went to doctor and got antibiotics. The fever went away but the ulcer is curing slowly. I personally think that is he has Diabetes which started to manifest because of the infection, but I'm only a third year student with a long way to go so now.... What do you think?
Going by the history and presentation alone, it could be a case of osteomyelitis in my opinion, however there are other conditions and risk factors too. Like his immunization status for tetanus is important but tetany is uncommon nowadays. So rusty nail puncture wounds can cause pseudomonas osteomyelitis. Diabetes, Low blood supply, neuropathy etc are the risk factors. Diabetic ulcer do not present with fever, so it points towards diagnosis of osteomyelitis, he should be treated with antibiotics and pus should be sent for culture & sensitivity, and treated accordingly.
