What Causes Back Pain Along With Abdominal Swelling?

I have been having a lot of stomach and back pain and swollen stomach. stomach pain is middle just under rib cage and radiates out to both sides. most of back pain is on the right ride. also have lower abdominal pain. hurts to have anything touching stomach like pants and underwear bands. I am 57 years old
