Whenever I m passing motion I feel backache that going to both sideways which increases after coming out from loo. I am a patient of chronicle liver disease, my report says I m suffering with mild umbilical hernia plus my spleen size is 20 cm and platelet count is around 100000

This is common in may patients specially when you are prone to gut infections, as in your case, sometimes rectum gets infected and you feel pain while defecation , i would suggest you to discuss this with your gastroentrologist and discuss about possibility of starting a proper antibiotic
I would also suggest you to use ample fluids and high fiber diet.
