What Causes Bad Odour From Patient?

I was with someone in the hospital. I smelled diarrhea when the door was opened. the smell was very foul around the bed. when the patient talked the smell was overpowering. I thought she had lost control of her bowels in the bed. can you tell me what could cause this.
