What Causes Black Out Followed By Fainting?

For awhile now I have had two episodes where I blacked out and fainted. Both times i knew it was going to happen as I started sweating followed by numbness in my hands and feet. Also my vision was as if everything started getting smaller and smaller before lights out and I would collapse. Have been having tension in my neck and the back of my head. I am almost always dizzy and light headed with tension throughout my whole body, numbness in my hands and feet along with hot and cold sweats. My doctor keeps saying it is high blood pressure and high cholesterol ( both are very high) but even when I eat right and exercise I still have these symptoms. It is these symptoms that cause me to be highly irritated and moody. I am constantly getting the shakes that go along with these symptoms. To make it even more fun, all these symptoms are causing me to constantly be exhausted. When I move my head to the sides I feel a popping sensation that relieves these symptoms but only for a few seconds. What can I do to get this to go away? This has been going on for years now and I can no longer take it. It is having a major strain on my lifestyle now.
