Hi each time I try to clean my daughter s ears we struggle, so today as I struggled to clean hers dirt was coming out stuck on cotton bud and I tried inserting it again as she was screaming and could not sit still now the bud had a bit of blood. I cleaned her ear with a wipe and theres no more blood and she says its nit painful. She is 4 years old, do you think this needs medical attention right away?

I would suggest you to to stop cleaning wax for the time being and give some time for soreness to heal. Use antibiotic ear drops to prevent infection accumulating there. Visit ENT specialist if your child experiences further bleeding or pain. Get wax removed with microscopic instrument. Maintain basic hygiene. Do not stress.
