What Causes Blood During Urination?

I just noted some discoloration of blood tinged urine. I had a total hysterectomy 3 years ago and has never experienced any bleeding. I still have my ovaries. No pain or discomfort just bleeding with urination which is also evident on wipes used afterwards.
