Hello,
The source of the bleeding can be either the vagina since she had some itching and could have scratched, or from the cervix if the membranes have separated. Dilation of the cervix to one finger is often found at term in a multigravida who has had one or more vaginal deliveries. This can often happen even when the person is not in labor. Yet, since there is some hint of an onset of early labor, you may report to your consultant for a clinical evaluation and the need for advancement of surgery. Take care.
