Nerine is an 82-year-old woman who is scheduled for a laparoscopic cholecystectomy for recurrent cholecystitis and cholelithiasis. Nerine is an active individual who walks 5 miles a day, does water aerobics three times a week, and belongs to a square-dancing club. She has a history of hypertension and right carotid stenosis. Her surgical history includes two cesarean sections and a right shoulder replacement. Nerine was taken to the OR as scheduled. She tolerated the anesthesia induction and endotracheal intubation without incident and was hemodynamically stable. Shortly after the surgeon initiated insufflation, the anesthesia provider announced that Nerine was bradycardic. The perioperative nurse noted that Nerine had a heart rate of 30. What is the likely cause of Nerine’s bradycardia?

According to my long surgical experience, the most common cause of bradycardia in a patient after insufflation of intraperitoneal cavity is vagus nerve mediated cardiovascular reflex after the raised intra abdomen pressure with CO2.
However particular case should be discussed out for the same with the concerned anesthetic only, who was present during the surgery.
