What Causes Breathing Difficulty, Drowsiness, Heaviness And Numbness In Arm?

Im not sure if I need to go to the ER- I have diff breathing , very drossy, my left side of arm feels heavy and knumb like I slept on it my chest wall feels like I bruised it or I lefted something heavy. I have hx of mitral valv regurgitation and enlarged heart
