General & Family Physician 's  Response
Hello,
Thank you for the question
I recommend that you go to the emergency department immediately since Coronary artery disease need to be ruled out especially since you are a male above 50 years. The pain you are describing typically associated with angina.
More over your risk increases with history of diabetics , hypertension , peripheral vascular disease and positive family history.

I RECOMMEND THAT YOU TAKE 2 ASPIRIN 75 MG TABLETS IF AVAILABLE AT HOME BEFORE GOING TO THE EMERGENCY ROOM.

At the emergency room you will need a ECG taken ,started on antiplatelet and anticoagulant medications. If ECG comes normal, you will need to undergo exercise stress test and further work up including angiography.

Hope I was helpful
regards.
