Hello and welcome to HCM,
Tightness in chest, unproductive spasmodic cough, fever and fatigue are suggestive of recurrence of pneumonia.
A clinical assessment is required.
Since, the cough is unproductive, there is possibility of atypical pneumonia like pneumonia caused by mycoplasma, Legionella etc, thus the symptoms will not respond to conventional antibiotics.
Thus, treatment for atypical pneumonia is required for which you need to consult your physician for clinical assessment and relevant investigations like X-ray chest.

Thanks and take care
