Hello and Welcome to ‘Ask A Doctor’ service. I have reviewed your query and here is my advice.

It seems that the searing burning sensation on your shoulders and lower neck and then in your forearms associated with nausea and fatigue could be due to arthrosis of the spinal cord. Further investigations should be run to rule out possible cardiac problems including:

- MRI of spinal cord
- cardiac check up: EKG, echocardiogram ,etc.
- complete blood count
- lipid profile testing

Hope I have answered your query. Let me know if I can assist you further.
