What Causes Constant Lack Of Concentration In A Child?

My 12 year old son is having severe difficulty testing and focusing in school. Last year I thought at first that it was just him being a boy. But today he broke down in tears when I talked to him about his failing grades and he said it's just too hard to understand it all. Is there a possibility that he may have attention deficit disorder?
