What Causes Constant Pressure In The Ear?

Hello. For the past 9 months or so I have had a constant pressure in my right ear. I try swallowing, but it does not pop the ear, as normally it would. During exercise, especially once my heartbeat gets up and I am breathing heavily, it feels like a balloon expanding and contracting within my ear. I have already had a check-up from my physician / doctor, and an exam by an Ear Nose Throat doctor on the outside of my ear. I think it is coming from the inside. I punctured my right ear drum a few years back, but it has already healed and scarred over.

Constant pressure in the right ear for 9 months, feeling of balloon expanding and contracting on exercises and the heart beat rises, on a healed and scarred ear drum.
The possible causes are Eustachian tube dysfunction or the problem in the middle or the internal ear.
This can be diagnosed on an ENT endoscopy.
Get another opinion, and go for high resolution CT scan scan of the ears.
