What Causes Crawling Sensation In The Penis And Anus?

I have been having a crawling in my anus and my penis for a very long time now, after I did appendicitis in 2010, it pains,bites and discharge whitish colour particle a times, please I need help because this thing has reduce the size of my penis to that of a 2years old boy s own. Please help me overcome this challenge. Thanks.
