What Causes Delay In Menstruation Other Than Pregnancy?

hi.. I have PCOS, but my periods were fairly regular in the past few months. I mean my date always came about 10 days late. But this time on the 39th day of my cycle i experienced very very light spotting for 3 days. It was only in the tissue when i cleaned that area and then nothing after that for the whole day. My home pregnancy test came negative. I got an ulttasound done n thats too a negative. But i still havent got my period n its been about 45 days since my last period. My ultrasound says that the Endometrial echoes are midline (10.7 mm). Both ovaries are normal in size and volume. Multiple immature follicles ranging from 5 to 8mm are present in the periphery of both ovaries. No adnexal mass is present. What does all this mean? is it possible to tell from the thickness when will i get my period? and why did the spotting occur? We are trying to conceive. Please advise.Im really confused and worried. Thank you.

Hello,
Thanks for sharing your health concern with us. I have gone through your query and would like to offer some suggestions. Your sonogram is evidencing polycystic ovarian picture. There is no evidence of ovulation and your abnormal menstrual pattern is mostly a breakthrough bleeding. The endometrial thickness is not helpful to predict the onset of a period. I would ask you to have a complete hormone assay, AMH titres, antral follicular count, serum DHEAS levels along with blood counts and urinalysis. Please maintain a healthy lifestyle. Further management can be planned accordingly. Hope this helps.
