What Causes Diarrhea With Mucus Covered Stool?

Hi, I've been suffering with infertility due to pcos and a month ago went a poo with lots of blood clots and fresh blood.. I went to the hospital and they couldnt find anything. For the last 3 days I've had severe diarrea and when I just went to the toilet now my poo was covered in a clear slime. HELP
