What Causes Dizziness, Fatigue, Frequent Urination And Sleeplessness?

I have been experiencing what can best be described as waves of dizziness, (not vertigo), general fatigue, confusion, frequent urination, and difficulty sleeping. I did have back surgery in 2013 - for a herniated disc. Should I mention that my hometown is Ferguson, Mo, where my family home of over 50 years is located?

I really appreciate your concern, if this is not the vertigo then it may not be anything pathology like syndrome but it could be nonspecific neither it is due to past history of surgery some time it is due to age and gender which is not give here, hope this information helps, take care and have a nice day.
