What Causes Dizziness While On Medication For Hypertension?

I was recently put on medications for high blood pressure. My blood pressure is 137/78. However, for the last couple of days, when I stand up I almost pass out. When I sit down it goes away. When I was in the ER before, they told me that my blood pressure drops when I stand up. I had to leave work today ,because I need to stand for my job, and I was too light headed to stay. I took my blood pressure and it was 137/74. Then I stood up and took it again. It was 93/62. What causes this?

General & Family Physician 's  Response
Hello,

First of all, I want to know on which medication you are on and their doses. Besides are you suffering from essential hypertension or there is other pathology which is causing hypertension (like renal artery stenosis).

Some antihypertensive medicines like calcium channel blocker and nitrates can cause first dose hypotension. This type of hypertension is called postural hypotension, occurs only when position changes from sleeping to sitting or sitting to standing.

It can be avoided by starting medication with lowest therapeutic dose and increased gradually if response is not there. Consult physician and if required decrease the dose or stop one of the medication.

Hope I have answered your query. Let me know if I can assist you further.

Regards,
Dr. Hiren Hirpara
