Hello i have a made up make (im in nursing school) and i need to find a diagnosis. he is 60 yr old, with hx of hypertension and heart failure, he has complaint of dyspnea. States he needs to sleep with 4 pillows at night, states he gained 6 lbs in 2 days. Also complains he is fatigue all the time, and has large amount of pink-frothy sputum with coughing. He is having difficulty expressing his wished because he is so short of breathe and coughs every time he speaks; cough has audible wheeze, skin is pale, and purple colored around his mouth. BP: 110/50, P:110/ a min, RR:32/ min, SAO2:86% temp:96.5, Weigth:240.Resp assessment: he has rapid breathing, crackles all lobes, but especially in bases bilaterally, shortness of breath, coughing with episodes of paroxysmal periods of dyspnea, orthopedia, and audible wheezes heard within the lungs in all lobes with auscultation.Cardio assessment: dizziness, weakness, grunt-snores sounds especially heard #4 and #5 intercostal areas. complains of tightness midsternal, pain level 5/10.what would you suggest as a diagnosis?
