What Causes Ear Infection And Hearing Loss After Taking Hydroxychloroquine?

Hi there, I have been diagnosed with an auto-immune disease (underlying tissue disease/mild RA). I have been put on Hydroxychloroquine (anti inflammatory medication). Just before I started taking the medication I have developed a really bad ear infection and lost hearing in one of my ears for nearly 2 weeks, it has then moved to my chest and I ve had an awful cough/out of breath/wheezing symptoms for a week now. It seems to be clearing up but now I have awful pains under my right rib cage (presume it s liver?). I had an ultrasound done some time ago and all organs seemed ok, I do however had some extreme levels of ALT in the liver tests for the last couple of years but no action has been taken on that by my doctor. A few questions here: 1.     I know my medication can potentially damage liver in the long term? How safe it really is? I know they treat Malaria with this type of medication – I was told that I would very likely need to take it for the rest of my life. 2.     The medication shuts down the entire immune system, and I am worried now that I will have constant infections and colds just as I had over the last few weeks since stating the pills. Is there any way to protect myself against this while on the medication? I cannot be constantly sick and this has been the case for a little while now. 3.     I had a history of abnormal liver tests however never all at once, just a few of them hence no action was ever taken by my doctor. E.g. the ALT test which I believe relates to the auto-immune disease - always double/triple in number to normal range. Is this something I should be worried about? I don’t want to take my medication and then find out that it even further damaged my liver? 4.     For the last few years I had awful symptoms and struggled so much with my health (at the age of early 20s!), it seems that this auto-immune disease has damaged my digestive system, cardiovascular system and so on. I have been reacting to a lot of foods, I have developed Reynolds syndrome. I am now on special diet, which helps but I wondered if there is any way of reversing auto-immune problems like this? So I don’t have to take this medication long –term and perhaps heal naturally? One thing to note is, the medication seemed to help with some symptoms, the swollen joints and pain in muscles. I know it’s a lot! But it’s been such a struggle to work all this out and took so long, and considering I am only in my early 20s I really want to make it to go away. Thank you. Mag

Rheumatoid arthritis is an autoimmune disease that mainly affects joints. As you already know about the medication, Hydroxychlorquine can cause:
1) Hepatobiliary disorders: Liver function tests abnormal, hepatic failure acute
2) Ear and labyrinth disorders like tinnitus, nystagmus, vertigo and hearing loss but not ear infection
3) Nervous system disorders: headache, dizziness, seizure, ataxia and extrapyramidal disorders such as dystonia, dyskinesia, tremor

But many patients who have RA have been using Hydroxychloroquine with very minimal side effects. As you already have liver enzymes elevation, you must be cautious and keep checking with liver function tests frequently. Please do inform your doctor about your liver function tests.

Coming to hearing loss due to infection, it's only temporary but with usage of Hydroxychloroquine, it may be significant and may become long lasting hearing loss. Please do inform your physician about your frequent upper respiratory infections. Ask him to reduce the dosage s of drugs used for treating RA.
