1) Your age
2)Duration of diabetes and current
treatment of diabetes.
3)Any associated conditions like
high blood pressure, cholesterol problem, heart problems and medicine for it if any.
4)Any habits like smoking and alcohol.
5)Any associated symptoms like puffiness of face, decreased urine volume, breathing difficulties while walking or at rest.
There are following possibilities in your case
1) Medicine or drugs- certain medicine for diabetes and blood pressure like pioglitazone, amlodipine.
2) Heart disease - Poor pumping capacity of heart (due to
diabetic cardiomyopathy or after heart attack). In this situation swelling is usually associated with breathing difficulties on mild exertion.
3) Kidney disease -
Diabetic nephropathy may be associated with loss of
protein in urine which causes hypoproteinemia that leads to swelling.
In nephropathy there might be water retention in the body which may cause swelling.
4) Anemia ie low hemoglobin- This could be nutritional or due to nephropathy.
5) Liver problems.
6) Diabetic Foot- It is usually due to blood vessel complications and
diabetic neuropathy and it is usually associated with shiny red overlying skin with severe pain.
I will advise you to do following investigation which might help in diagnosis.

Complete blood count.
* Liver function test.
* Kidney function test.
* Urine Examination.
* 24 hours urine protein.
* 2DEcho of heart.
* Ultrasonography of abdomen (for liver and kidney)
High ESR indicates long term inflammatory process in the body. In Diabetes it may be falsely elevated.
