Hello and welcome to HCM,
The test report shows an increase in TSH levels.
TSH levels are controlled by levels of T3 and T4 in blood.
Decrease in level of T3 and T4 is called hypothyroidism and causes increase in TSH levels by negative feed back.

Your history suggests that you are suffering from hypothyroidism.
In hypothyroidism there is fall in levels of T3 and T4.
Hormone replacement with thyroxine is required to replace the low levels of thyroid hormones.
The dose of thyroxine that you were taking is insufficient to maintain normal hormonal levels.
Thus, the dose of thyroxine has to be increased to maintain the levels of T3 and T4.
After increasing the dose of thyroxine, thyroid function tests have to be repeated at regular intervals.

Thanks and take care
