Hi,I am Dr. Subhankar Chakraborty (Gastroenterologist). I will be looking into your question and guiding you through the process. Please write your question below.
What Causes Elevation In Liver Enzymes With Low Platelet Count?
HI! My Father is 79 years old with fever for 3 days and epistaxis. He has been admitted in the hospital and lab test have done, yielded with the following results:
ALT : 2989 u/L
AST: 6760 u/L
Total Bilirubin: 96 umol/L
Platelets: 14
The Doctors were saying that it is Primary Viral Hepatitis but after those results and other investigations, it wasn't. Now we're a little confused with the exact diagnosis. Could you please help me... I'm willing to send more information / data you might be needing to rule out this case. Thank you!
These are extremely worrisome numbers. I'd say at this point, it doesn't really matter what the cause is, but the end result is that your father is in florid liver failure. He had epistaxis (nose bleed) likely because his platelets and his liver enzymes that make clotting proteins are not working.
If his total bilirubin is truly 96, then that is the highest number I have ever seen, and to be quite honest, I'm surprised he's still alive. At this point, he likely needs a liver transplant urgently, and the cause doesn't really matter.
I hope this answers your questions. Please feel free to contact me should you have any further questions.
