There are several options for oral drugs for people with type 2
diabetes mellitus, but the most commonly used are:
Biguanida group that is
metformin.
Second generation
sulfonylureas (SFUs) are: glyburide (glibenclamide),
gliclazide,
glimepiride, and glipizide.
From several studies about the second generation sulfonylureas mentioned that:
Compared with other SFUs, glyburide has been associated with an increased risk of
severe hypoglycemia, especially in the elderly. Evidence show the increased
relative risk of hypoglycemia and the resulting harm with use of glibenclamide versus any of the other second generation SFUs, particularly gliclazide and glipizide. The data unequivocally recommends against the use of glibenclamide in older people.
Based on a review of safety, efficacy, cost, and NEML (Nasional Essential Medicines List) availability of glibenclamide, gliclazide, glipizide, and glimepiride, the recommendations are as follow:
1. Glibenclamide 2.5 mg and 5 mg tablets should remain on the EML (Essential Medicine List) with age restriction recommending against use in people older than 60 years of age.
2. Gliclazide 80 mg tablet should be added to the EML for use in the older with type 2 diabetes, indicate that other second general SFUs (other than glibenclamide) are an acceptable alternative.
My advice is that you can choose another SFUs class medication, suitable for your age.
