General & Family Physician 's  Response
Hello....,
Looking at the history of your history, it appears your brother is having Diabetes mellitus.
As you may be aware it is condition in which one has increased blood sugar along with symptoms of polyuria(excess urination) polydipsia(excess thirst) and polyphagia(excess hunger).
Therefore to rule out DM you need to go for FBS(Fasting blood sugar), RBS (Random blood sugar) PPBS( Post prandial blood sugar). Basing on the test results DM can be diagnosed or other differential diagnosis can be concluded.
About his symptoms of foot, it is suggestive of peripheral neuropathy.
Please consult a local physician to confirm to a diagnosis.
Best Regards
