What Causes Fatigue Along With Shortness Of Breath And Distended Abdomen?

I have been coughing up blood for 2 months. My hemoglobin is 7. I was a heavy smoker but quit 4 years ago. I am an alcoholic. I have been fatigued, short of breath, my abdomen is distended. Up until recently the only thing I have been diagnosed with is hypothyrodism. I am weak and have fallen a few times.
