What Causes Fatigue Along With Stiffness In Neck And Shoulders?

I have severe fatigue as well as stiff neck with neck fatigue, pain in shoulders and upper back and headache as well as tightness in chest and shortness of breath. My doctor did an EKG, an echocardiogram, chest x-ray and labs including tests for various tick diseases including lymes and a blood culture. All tests were within normal except a muscle enzyme test was somewhat elevated. My doctor ordered a repeat of the lymes, etc. tests, put me on an antibiotic (doxycycline) and will be doing additional tests. I feel he is being thorough in trying to diagnose this. What would you advise?
