I have reviewed your query and here is my advice.
Symptoms of fluid in abdomen, cough, mild fever,
blood in stool, loss of weight and appetite, x-ray showing one lung and
ultrasound showing fluid and
liver damage are self-explanatory.
There may be liver
cirrhosis or secondary cancer in the liver with primary in the lung hence one lung is not seen on x-ray as there would be
hydrothorax.
I would advise him the following in such a situation:
• Get admitted.
• Further investigations of CT scan of the lung.
• Tests of blood, stool, and urine.
• Clinical evaluation and further FNAC of the lung, fluid analysis from chest and abdomen, FNAC of the liver and as would be advised.
This will give the proper diagnosis and a plan for further management.
