What Causes Frequent UTIs?

I am prone to UTIs, particularly when I drink water away from home. I returned from Thailand in mid-October and finished another 30 day prescription of Doxycycline for malaria prevention. As soon as I got off of the Doxy, the UTI showed up. My family doctor prescribed macrobid after a urinalysis and culture. I still had the pain after the macrobid, so he prescribed Cipro. The pain still did not go away, but another culture shows that there is no infection. He has referred me to a urologist, but I was trying to do some research to better understand what is going on. It feels as though I always have to urinate, or that I can literally feel the urethra. Intercourse is also painful, which has not been the case prior to this UTI.
