Hi,I am Dr. Shanthi.E (General & Family Physician). I will be looking into your question and guiding you through the process. Please write your question below.
My husband had a grand mal seizure tonight in th ER , he had passed out at home early in the afternoon and called me at work to ask me to come home I took him to the Er and he takes all his Medication on time. He takes keppra 1000mg bid. Why would this happen? He hasn t had a seizure in years.
Your husband is already on anti-convulasant Keppra (Levetiracetam)1000mg bid. Hence it could be a break-through seizure. Thare are certain factors which can precipiate seizure like sleep deprivation, alcohol intake and at times stress. Most of the time recurence of seizure is due to non- adherence (missing dose). Levetiracetam can be given maximum 3000 mg/d in two divided doses. Monotherapy should be the goal whenever possible. If seizures continue despite gradual increases to the maximum tolerated dose and documented compliance, then it becomes necessary to switch to another antiepileptic drug.
Hope I have answered your query. Let me know if I can assist you further.

Dr. Tushar Kanti Biswas,
