General & Family Physician 's  Response
Hi,

Spotting can be due to hormonal irregularities. As you having head ache and back ache other conditions should be ruled out. It can be due to infection or myoma or endometriosis. A pelvic examination by gynecologist and ultrasound examination should be done to know the exact status.

Take care. Hope I have answered your question. Let me know if I can assist you further.

Regards,
Dr. Nishad B. N., General & Family Physician
