What Causes Headaches, Dizziness And Blurred Vision?

I have had a car accident 4 years ago with many syptoms including severe headaches, severe head pain jaw pain, dizziness, blurred vision, memory loss, seveer neck pain, seveer back pain and seveer pain at the back of my head. I was thrown hard to the left then a awoke hearing a loud noise and then the car was thrown hard to the right. Now a MRI scan shows up Minor Mucosal thickening in left maxillarysinus and anterior ethoidal air cells. Could this be caused by my head being severely shaken?
