What Causes Heart Attacks?

i am asking for a friend, very new to steroid use, 22yo 160lbs 5 7 very high metabolism, is very athletic to begin with, occasionally smokes cigarettes (less then 8 per day), eats 50/50 healthy and unhealthy. what are the chances of having a heart attack on a Sustanon 250 cycle at a 500 dose per week? and if there is a chance of heart attack what would be the reasons why?
