I am 45 year old and for the last 4-6 months my periods have been heavier and more painful than usual...I have had several episodes of overflow and lots of large clots being passed....is this a normal part of premenopause or something more to be concerned with

Painful menstrual cycles need to be evaluated. I advise you to get ultrasound done to rule out fibroids of uterus or adenomyosis which causes painful menstruation. It is not a premenopausal symptoms.
