What Causes Instability And Imbalance In The Body?

My husband is 77 yrs old, obese, non smoker. He is having problems with stability when trying to move about. He feels unstable and can t control the movement of his legs very well. He has been doing physical therapy for over a year and not seeing much improvement. He has lost a lot of strength over past couple years. He is dieting and has lost 30 lbs but that hasn t improved any of the above.
