General Surgeon 's  Response
Hi,

Noted your history of pain in abdomen, Cholecystectomy 10 years ago, the pain in abdomen increases with constipation and is referred to back. The possible causes I can think about are as follows:
Problems with pancreas can cause similar pain.
Any retroperitoneal structure like lymph node enlargement can cause similar pain with referred pain in back.
Gastric ulcer on the posterior wall of stomach.

Hence I would advise you the following in such a situation:
Consult a General Surgeon for proper clinical evaluation, examination and investigations as follows:
X-ray of abdomen, Ultrasonography of whole abdomen.
CT scan of abdomen.
Tests of stool, blood and urine.

All these will lead to a proper diagnosis and thus allow a proper management as per the cause found. In the meantime, take a PPI like Pantoprazole, symptomatic treatment with antispasmodic or anti-inflammatory medication as needed.

Hope I have answered your query. Let me know if I can assist you further.

Regards,
Dr. T. Chandrakant, General Surgeon
