Hi,
Considering your symptoms are you sure it's just a skin tag? Skin tag is least likely to be felt prominently(impulse on) while coughing. Since you said you are babysitting your grand children so i presume you are well past your menopause. Taking all these into consideration what it indicates to me is that the chance of genitourinary prolapse needs to be ruled out first. This will need a proper physical examination and you should visit a local gynaecologist.
