What Causes Light Spotting And Mild Abdominal Cramps Despite Having Negative UPT?

Period is 4 days late, I ve only had very light spotting for last 2 days, not even enough for a tampon. I had mild cramping the day after my period was due, but nothing like my usual period cramps. Husband and I have been trying for a couple years (had some infertility issues and now I take metformin to regulate my hormones). My periods are almost always 29 days now that I take metformin. Had sex at optimal peak ovulation as I use OPK to monitor my LH surge. Took an Internet cheapie hcg test the last couple days, both negative. I m 37 and wondering if I might be going into peri menopause.

Hello,

As per my clinical experience, as periods are almost always there at 29 days, the issue of peri menopause does not sound appealing here. There can be ample number of spotting and abdomen cramps with negative UPT as:
- Hormone axis alteration or imbalance at hypothalamus, pitutary, ovarian levels
- Polycystic ovarian syndrome
- Altered LH FSH ratio
- Lack of estrogen, progesterone
- Low hemoglobin (anemia)
- Uterine fibroids

It is primary need of OBGYN check up with lab results, ultrasound of the abdomen and pelvis as primary care of concern to initiate the diagnostic approach.
