What Causes Loss Of Speech After A Car Trauma?

My friend was in a car accident 5 days ago. He was not wear a seat belt and hit the windshield. He presented with a featured c2 c3 and l3 l4. He had internal bleeding spline was cortorized to stop bleeding also staples in top of his head. He was lucid in the icu and for the first 3 days, but now he does not speak at all and is making a repetitive grunting sound.
