As per the history provided, you are 77yrs old, with a
Renal infection, presently on antibiotics (for 6 days, you have not mentioned the type of antibiotic) and hypotensive (in a background of
hypertension). I would like to enquire, whether you are diabetic or not, as
hyperglycemia can aggravate any form of infection in the body.
The symptoms put forth by you, could be in favor of an impending sepsis, specially
hypotension, in a borderline hypertensive patient.
Kidney infection (specially
pyelonephritis) are notorious for causing sepsis. In this scenario I would recommend you to visit your physician immediately. You may need certain investigations like Total leucocyte count/differential count,
C-reactive protein (infection markers), urea/creatinine (renal functione tests) and urine routine examination/culture (though it is highly possible that your culture reports may be negative, since you are already on antibiotics). In case you take antihypertensive drug please withold it for the time being (till your BP readings are regularised). Please consume plenty of fluids. If you are not responding to this particular antibiotic, it may need to be changed to a higher group.
Your treating physician, can examine you thoroughly, and give you a plan of management.
