What Causes Low Calcium And High Phosphate Levels Leading To Fits In Infants?

My son is 2 months old and he is facing problem of low calcium. His calcium reached to 5.4 and we faced problem of fits, then we admitted him to doctor and from last 12 days they are giving him calcium by injection and feed. Now its reached upto 7 which is very slow improvement. We have done TSh test also its normal. Phosphate level was high it was 361. We tried to search the other reasons also and doctor suggested PTH test and in PTH report the value of PTH was 191. Please suggest, Why our son is facing this issue. How we can solve it, if needed I can upload or send all test reports also.

Pediatrician 's  Response
Hi,

Basically he is having hypocalcemia, along with high PTH level- this happens in a condition called "pseudo-hypo parathyroidism", although before concluding, I would have ordered few other tests also- serum magneium, serum albumin, renal function test and urine calcuim-creatinine ratio.

In this condition, even though our parathyroid gland secretes PTH hormone as normal, it is not able to act in our body because of abnormal receptor. There are chances of skeletal abnormalities like facial dysmorphism and hand/finger defects in some patients.

In this case, response to calcium injection is delayed. After getting normal calcium level, we can switch to oral calcium, along with calcitriol(active form of vitamin D). Usually treatment is needed for long. Your doctor will discuss these details with you.

Hope I have answered your question. Let me know if I can assist you further.

Regards,
