Hard swelling over lower back or sacral area with severe pain that get aggrevated by even slight touch and movement is suggestive of some infection . Most commonly it's infection of hair follicles . That we call folliculitis . This can be managed by topical application of some antibiotics ointment and if severe you may need to take oral antibiotics . If it is not subsiding you should consult a dermatologist .
