General & Family Physician 's  Response
Hi,
From history it seems that there might be having developing direct inguinal hernia.
Due to increase intra abdominal pressure while doing defecation there might be having protrusion of intra-abdominal contents through lax abdominal muscles or lax inguinal ring.
This might not seen on CAT scan.
Consult surgeon and get physically examined.
Avoid constipation.
Do abdominal recti muscle exercise.
Ok and take care.
