Hi..
Welcome to HEALTHCARE MAGIC..

I have gone through your query and can understand your concerns..

As per your complain a small lump infront of the ear seems to be probably due to Pre-auricular Lymphadenitis that occurs due to swelling of the lymph nodes and the most common cause for it is infection in head and neck region..
But a lymph node swollen for 2 years needs to be evaluated as such a long duration can be due to either chronic infection while can also be due to lymphoma formation...
I would suggest you to consult a General Surgeon and get evaluated and a thorough clinical evaluation and investigations like Biopsy and ultrasound of the lymph nodes can help in diagnosis and treatment can be done accordingly...
For infection antibiotics and alternate warm and cool compresses can help..
For infected tooth consult an Oral Physician and get evaluated and a thorough clinical evaluation and investigation like x-ray can help in diagnosis and treatment can be done accordingly..
You can be advised either root canal treatment of tooth or extraction followed by antibiotics and painkillers for resolution depending upon condition of tooth..

Hope this information helps..

Thanks and regards.
